Terms & Conditions – $20 Amazon Gift Card Offer

  1. Eligibility

    • To qualify for the $20 Amazon gift card, participants must:

      • Sign up for a Nourish appointment using the official Amax Fit referral link.

      • Complete their first scheduled appointment with a Nourish Registered Dietitian.

  2. Redemption Process

    • After completing your first appointment, you must submit the Gift Card Request Form found HERE

    • Only completed forms will be processed for gift card delivery.

  3. Limitations

    • One (1) gift card per eligible participant.

    • Offer is non-transferable and cannot be combined with other promotions.

    • Gift cards will only be issued to participants who use the Amax Fit referral link at the time of sign-up.

  4. Gift Card Delivery

    • Upon verification of eligibility and receipt of the completed form, Amax Fit will send the $20 Amazon gift card electronically to the email address provided within 14 business days.

  5. Additional Terms

    • Amax Fit reserves the right to verify participant eligibility before issuing the gift card.

    • Amax Fit is not responsible for invalid, incomplete, or inaccurate submissions.
